Constant Care Health is seeking full time LVNS, LPNS, RNS and LCSW's with a passion for nursing and improving patient outcomes. As a telehealth nurse you will be responsible for a group of patients that you will provide support to on a monthly basis. You will be responsible for building and maintaining a rapport with your patients while coaching them to improved health.

· Provide education and coach patients on strategies for self-management to keep patients out of the hospital.

· Connect the patience with community resources when needed.

· Utilize our chronic care management software to call patients and to aid and support them through their current patient care plans.

· Implement and improve the current plan of care by updating medications, symptoms, appointments, biometrics, and additional interventions made.

· Close care gaps by assisting and communicating preventative care techniques.



Required Skills/Education

· Strong Communication and Telephonic Skills

· Excellent Organization and Time Management

· Self-Starter

· Passion For Nursing and Client Care

· Proficient with a EHR system

· Good with Technology

· Open To Learning New System

· Current Multistate Licensed LVN/LPN/RN
